PHC offers Intensive Case Management through two programs: New Directions, and the Santa Clara County Full Service Partnership.
 

 

 

ABOUT NEW DIRECTIONS

PROGRAM MODEL

New Directions serves the highest cost and highest need patients in the hospital system, employing a “whatever it takes” recovery model when working with patients.  Small caseloads allow case managers, who are masters level educated and trained social workers, to work intensively with each patient or family, mutually establishing a plan and goals to increase self-sufficiency.

ABOUT SANTA CLARA COUNTY FULL SERVICE PARTNERSHIP (FSP)

Referrals to FSP are made exclusively by the Santa Clara County Mental Health Department.  Clients served receive a range of services, including intensive case management, rehabilitation, and medical support for adult clients.  The ultimate goal of this program is to enable patients to maintain their mental health and successfully reintegrate into the larger community.
